RN Case Manager

Gervino Group is working with a dedicated hospice care organization to find an experienced RN Case Manager who leads with both clinical skill and heart. This role is about more than care coordination—it's about showing up for patients and families during some of life's most meaningful moments, while guiding a nursing team you can be proud of.

What you'll be doing

• Managing patient care across hospice settings with compassion and clinical precision
• Coordinating with interdisciplinary healthcare teams to keep care seamless
• Providing leadership and day-to-day support to nursing staff
• Covering a territory centered on Ambridge, PA, with a 15–25 mile radius

Schedule
Monday–Friday, 8:00 AM–5:00 PM · 6 units/day, 30 units/week · 1 weekend per month · 1 evening of on-call per week

Required qualifications

• High school diploma or GED
• Valid Registered Nurse (RN) license in Pennsylvania
• 1+ year of hospice experience
• 1+ year of management or supervisory experience
• 1+ year of experience in home health care, Epic Home Health, or a home health setting
• Current AHA Basic Life Support (BLS) certification
• Current AHA Advanced Cardiac Life Support (ACLS) certification
• Comfortable with regular local travel across multiple counties
