Our client is seeking a Hospice RN to join a dedicated, supportive care team focused on delivering comfort, dignity, and quality of life to patients and their families.

Monday–Friday, 8:00am–5:00pm
On-call rotation: one weekend every 5–7 weeks (Fri–Sun, 8pm–8am)
Two holidays per year

What You’ll Do
- Manage and deliver individualized hospice care plans
- Provide compassionate, patient-centered care in home settings
- Support patients and families through education and guidance
- Complete timely, accurate documentation using EMR systems
- Coordinate with an interdisciplinary team to ensure continuity of care
What We're Looking For
- Active RN license in Pennsylvania
- Associate’s degree or higher in Nursing
- 2+ years of RN experience (hospice experience strongly preferred)
- Experience with EMR systems (TruChart or similar)
- Strong time management and ability to work independently
- Valid driver’s license, reliable transportation, and auto insurance
