Physical Therapist - Home Health

What you'll be doing

• Collaborating with a care team on the management and delivery of patient care plans across Somerset and nearby communities
• Traveling to patients' homes (roughly 20% of your day)
• Keeping up with charting using state-of-the-art technology designed to keep it fast
• Self-managing your schedule with support from dedicated schedulers

Schedule
Monday–Friday, 8:00 AM–5:00 PM · 1 weekend per month · 2 holidays per year

Required qualifications

• Bachelor's degree or higher in Physical Therapy from an accredited institution
• Valid Physical Therapist license issued in Pennsylvania
• 1+ year of healthcare experience
• 1+ year of experience with EMR systems (including Practice Partner or equivalent)
• Valid driver's license and comfort with local travel to patient homes
• Comfortable with tablets, mobile phones, and digital tools as part of daily workflow
• Pennsylvania PT license number—please include it in your application if available
